Thunder Disaster Services, Inc. is currently working on preparing the 2006 Disaster Subcontract Agreements for the upcoming hurricane season. In order to do so, we need for you to complete the enclosed forms and fax or mail them back to the office as soon as possible.

The information above is requested to make sure that Thunder has the most current data on file before preparing a Sub-Subcontract Agreement; therefore a prompt reply is needed.

In addition to the above information, it is imperative that Thunder has a Certificate of Insurance completed in strict accordance with your subcontract requirements which are found in Article Six Insurance, your performance and payment bond, if required. The insurance information sheet included with this packet should be helpful in attaining the correct information from your insurance agency. If your company utilizes a leasing company for any of your employees, it is a requirement that your leasing company grant an Alternate Employer Endorsement on the workers’ compensation policy in favor of your firm.
